abstract = {Total neoadjuvant therapy (TNT) has reshaped the management of locally advanced rectal cancer (LARC), improving disease-free survival (DFS) and local control and enabling nonoperative management in selected patients. However, total mesorectal excision (TME) after TNT is technically challenging because therapy-induced fibrosis, inflammation, and adhesions can obscure anatomic planes. Robotic surgery, offering three-dimensional (3D) magnified visualization and wristed instrumentation, may mitigate these difficulties by enhancing precision and dexterity in deep pelvic dissection; however, post-TNT-specific randomized evidence for robotic surgery remains limited, and the clinical implications of robotics in the post-TNT setting have not been comprehensively synthesized. This narrative review provides an overview of clinical implications of robotic TME after TNT for LARC.},
